1. Booking Policy
– Clients must be 18+ years old and present valid government-issued ID.
– A non-refundable deposit is required to secure all appointments.
– Deposit is applied to final price of the tattoo.
– Prices are based on size, detail, placement, and time required. All estimates are subject to change.
2. Cancellation Policy
– At least 48 hours’ notice is required to reschedule or cancel an appointment.
– Late cancellations or no-shows result in forfeiture of deposit.
3. Tattoo Procedure Requirements
– Clients must complete a Medical & Consent Waiver before any procedure.
– If you are pregnant, nursing, under the influence of drugs/alcohol, or have any condition contraindicated for tattooing, you will be refused service.
– Artists reserve the right to deny service for any reason related to health, safety, or conduct.
4. Aftercare Responsibility
– Clients are responsible for following aftercare instructions provided.
– Back Bay Ink is not liable for healing issues arising from poor aftercare or neglect.
5. Intellectual Property
All designs, photos, and digital content are property of [Studio Name] or the respective artist. You may not copy or reproduce artwork or content without written permission.
6. Limitation of Liability
By receiving services, clients acknowledge that tattooing involves risks including, but not limited to, allergic reactions, scarring, and infection. You waive all claims against Back Bay Ink, its artists, and employees except in cases of gross negligence or misconduct.
7. Massachusetts Tattoo Law Compliance
All services are performed in compliance with 105 CMR 123.000, MGL Chapter 112, and relevant health board regulations. All instruments are single-use or properly sterilized in an autoclave. Consent and age verification are mandatory.
By booking an appointment and signing the physical consent form in our studio, you acknowledge and agree to the following:
– You are 18 years of age or older.
– You do not have any medical or skin conditions that may interfere with the tattoo procedure.
– You are not under the influence of alcohol or drugs.
– You understand the nature of the procedure and the potential risks involved, including but not limited to allergic reactions, infections, or discomfort.
– You release Back Bay Ink, its artists, and employees of all liability except in the event of proven negligence.
